Swine flu H1N1 vaccine too late to help most

http://www.washingtontimes.com/news/2009/oct/20/study-swine-flu-vaccine-too-late-to-help-most/

Swine flu H1N1 spread

Swine flu H1N1 spreads through coughing and sneezing by others infected. Most people have had the normal flu several times in their life, and the symptoms are similar in either case: a fever of over 100 degrees farenheit, chills, stuffy nose, coughing, sore throat, aching body, tiredness. There are some differences however with Swine flu H1N1

Swine flu H1N1 in children, signs that indicate an emergent medical trip include difficulty breathing, acute vomiting, lethargy or not waking up, severe irritability, not drinking enough liquids, bluish or grayish skin tone, flu symptoms that decrease and then return with a worse cough and fever.

For adults, symptoms that indicate emergency medical help would include labored difficult breathing, severe vomiting, dizziness, pain in chest or stomach or pressure in those areas, confused thinking, and flu symptoms that diminish and then return with greater coughing and fever.

Swine flu H1N1: this respiratory disease does not normally result in swine to human transmission. This year a new form of the virus has developed and is now being passed person to person. The U.S. is not deteching the virus in any herd of pigs presently, but in Alberta, Canada, an infected herd was discovered and is now under quarantine. Some people are concerned about eating pork, but the virus is not spread through food. Egypt has decided to slaughter all their pigs, and other countries may have followed suit, though this has not become news yet. Pandemic flu is one in which there is very little immunity in the body, and the disease spreads quickly amongst people. There is no pandemic flu at the time of this writing. The H1N1 virus is being carefully monitored in order to determine how the virus is spreading. Seasonal or common flu is a respiratory illness that spreads person to person, and there is a vaccine for it. Most people normally have an immunity to some degree built up against the common flu. Precautions: have kleenex, hand sanitizer with an alcohol base, and wash with soap and water frequently. Employees should leave the work premises if they are showing symptoms of the flu.

Why are people focusing so much on Swine flu H1N1 compared to other flu? As in any flu, other medical conditions you may have can result in a person being more susceptible to the flu, and a combination of the two may be more dangerous. People with heart disease are more susceptible. In addition, because this is a respiratory virus, anyone who already has respiratory issues can be doubly impacted by this flu.

Having a flu shot will not protect your from Swine flu H1N1, otherwise there would be lineups of people waiting for their vaccination.

There is an anti-viral drug for swine flu or Swine flu H1N1 that is being provided to some patients, but usually the symptoms need to be within two days prior to having the drug. Health care departments are administering this anti-viral drug to the most acutely ill and those who have to be in close proximity to those who may have symptoms.

Who is most at risk for acquiring H1N1? People with respiratory conditions, especially those under age five, pregnant women and older people over age 65.

Ive been around a lot of people who came back from Mexico this past spring break. We all wash our hands at work and a couple staff worked from home for one week as their spouse or them was feeling ill. I have not become ill, nor have any of them tested positive though they have been tested a few times now just for safety sake.

Swine flu illegal drugs and cures: FDA is coming after you

FDA NEWS RELEASE

For Immediate Release: Oct. 15, 2009

Media Inquiries: Christopher Kelly, 301-796-4676 christopher.kelly@fda.hhs.gov
Consumer Inquiries: 888-INFO-FDA
FDA Warns of Unapproved and Illegal H1N1 Drug Products Purchased Over the Internet

The U.S. Food and Drug Administration today warned consumers to use extreme care when purchasing any products over the Internet that claim to diagnose, prevent, treat or cure the H1N1 influenza virus. The warning comes after the FDA recently purchased and analyzed several products represented online as Tamiflu (oseltamivir), which may pose risks to patients.

One of the orders, which arrived in an unmarked envelope with a postmark from India, consisted of unlabeled, white tablets taped between two pieces of paper. When analyzed by the FDA, the tablets were found to contain talc and acetaminophen, but none of the active ingredient oseltamivir. The Web site disappeared shortly after the FDA placed the order. At the same time, the FDA also purchased four other products purported to diagnose, prevent, treat or cure the H1N1 influenza virus from other Web sites.

These products contained various levels of oseltamivir but were not approved for use in the United States. Several of the products purchased did not require a prescription from a health care professional. Additionally, the products did not arrive in a timely enough fashion to treat someone infected with the H1N1 influenza virus, or with an immediate exposure to the virus.

“Products that are offered for sale online with claims to diagnose, prevent, mitigate, treat or cure the 2009 H1N1 influenza virus must be carefully evaluated,” said Commissioner of Food and Drugs Margaret A. Hamburg, M.D. “Medicines purchased from Web sites operating outside the law put consumers at increased risk due to a higher potential that the products will be counterfeit, impure, contaminated, or have too little or too much of the active ingredient.”

Consumers may not know exactly what they are getting when buying such antiviral products on the Internet from an unfamiliar company. Patients who buy prescription drugs from Web sites operating outside the law are at increased risk of suffering life-threatening adverse events, such as side effects from inappropriately using prescription medications, dangerous drug interactions, contaminated drugs, and impure or unknown ingredients found in unapproved drugs. This may particularly be the case in the event of a public health emergency, such as an influenza outbreak, where approved treatment options would be in high demand and expensive, and where drug shortages could occur.

Drugs that are in high demand are vulnerable to counterfeiting and diversion because buyers may be desperate to stock the product, and criminals capitalize on the situation.

The FDA urges consumers to only purchase FDA-approved products from licensed pharmacies located in the United States. Consumers should contact their health care provider if they have any questions or concerns about medical products or personal protective equipment.

The two antiviral drugs approved by the FDA for treatment and prophylaxis of the 2009 H1N1 influenza virus are Tamiflu (oseltamivir phosphate) and Relenza (zanamivir).

Tamiflu and Relenza, in addition to their approved label, have Emergency Use Authorizations that describe specific authorized uses during this public health emergency.

The FDA actively monitors the Internet, and where appropriate, will purchase and analyze drug products.

Consumers can also visit FDA's Web site for tips about how to protect themselves when buying medicines online.

Swine flu Vaccine side effects;h1n1 vaccine risks

"Swine Flu Vaccine Side Effects Mild, Say Researchers"

Separate studies by researchers from the United Kingdom and Australia on
the efficacy and safety of two vaccines for swine flu - both using the same
strain of the virus and standard manufacturing processes used for seasonal
flu vaccines but each made by a different company--show that about 75
percent of people receiving the vaccines in trials had produced antibodies
to the swine flu virus within 21 days after receiving a single dose of the
vaccine. In the U.K. study, which compared 1 dose with 2 doses, people had
higher levels of antibodies if they received both doses, but most people
had some level of protection within 14 days after the first dose. No
serious side effects were reported in either study. Common side effects
were those you'd expect from seasonal flu vaccine, including pain where the
vaccine was injected and muscle aches or headaches. A swine flu vaccine
will likely to be approved by the European Medicines Agency in October.
The U.K. government has said that people who are usually offered seasonal
flu vaccine due to their age or health problems will be given priority when
the vaccine arrives. Pregnant women may also be given priority, as long as
safety testing shows the vaccine is safe for use during pregnancy.

A new report suggests that swine flu remains contagious for about a week or

A major shortage of Swine flu H1N1 vaccine

Geneva - Production of swine flu vaccines will fall "substantially" short of the amount needed to protect the global population, the World Health Organisation warned Friday as the pandemic death toll rose.

"Current supplies of pandemic vaccine are inadequate for a world population in which virtually everyone is susceptible to infection by a new and readily contagious virus," WHO director general Margaret Chan said in a statement.
Despite new evidence that only one dose of the vaccines currently being tested will be enough for most people, WHO spokesman Gregory Hartl said output next year will be "substantially less" than the 4.9 billion doses annual production forecast.

Some 25 pharmaceutical laboratories working on vaccines have indicated that weekly production is lower than 94 million doses, he said.

In May, the WHO had forecast a weekly output of 94.3 million doses if full scale vaccine production was launched.

But pharmaceutical companies have in recent weeks slashed their production expectations due to poorer than expected yields from the so-called "seed virus" strains developed by WHO-approved laboratories.

Novartis swine-flu jab effective in single dose
Thursday , September 03, 2009

A study of Novartis' pandemic swine-flu jab shows it is potentially protective after just one dose, which could help eke out supplies for mass immunisation across the globe.

Other companies making the H1N1 vaccines, such as GlaxoSmithKline, have said two doses must be given as general practice for optimum protection from the virus, adding to fears of public health officials that supplies will be short.

But Novartis' first pilot trial of its investigational A (H1N1) with 100 subjects indicates strong immune response in 80% of subjects after just one dose, and more than 90% after two doses.

Dr Andrin Oswald, chief executive of Novartis vaccines and diagnostics, said the results for the vaccine, to be called Celtura, are encouraging.

He added: "The study suggests that while two doses seem to provide better protection, one dose of our adjuvanted Celtura vaccine may be sufficient to protect adults against the swine flu. This is important information for public health authorities who prepare for vaccination in the coming months with limited vaccine supply."

The trial was run by the University of Leicester and University Hospitals of Leicester in the UK, testing 100 healthy volunteers aged between 18 and 50.

It evaluated the tolerability and immunogenicity of the vaccine, and different schedules and timing between vaccinations were tested.

Additional pivotal clinical trials, with larger numbers of subjects and sponsored by Novartis, are already under way around the world, and they will include more than 6000 adults and children.

The aim of the trial was to find out how many doses and what type of vaccine is needed to give protection, and the initial results should help to plan vaccination campaigns in the autumn, including doses and timings.

Comparing H1N1 with 1918 flu which killed millions

The "Spanish" influenza pandemic of 1918–1919, which caused ≈50 million deaths worldwide, remains an ominous warning to public health. Many questions about its origins, its unusual epidemiologic features, and the basis of its pathogenicity remain unanswered. The public health implications of the pandemic therefore remain in doubt even as we now grapple with the feared emergence of a pandemic caused by H5N1 or other virus. However, new information about the 1918 virus is emerging, for example, sequencing of the entire genome from archival autopsy tissues. But, the viral genome alone is unlikely to provide answers to some critical questions. Understanding the 1918 pandemic and its implications for future pandemics requires careful experimentation and in-depth historical analysis.

Swineflu traced back to U.S factory farms

Scientists have traced the genetic lineage of the new H1N1 swine flu to a strain that emerged in 1998 in U.S. factory farms, where it spread and mutated at an alarming rate. Experts warned then that a pocket of the virus would someday evolve to infect humans, perhaps setting off a global pandemic.

FDA, FTC Warn Public of Fraudulent 2009 H1N1 Influenza Products
Offending web sites and illegal activity targeted for action

The U.S. Food and Drug Administration and the Federal Trade Commission are alerting the public to be wary of Internet sites and other promotions for products that claim to diagnose, prevent, mitigate, treat or cure the 2009 H1N1 influenza virus. The agencies are also advising operators of offending web sites that they must take prompt action to correct and/or remove promotions of these fraudulent products or face enforcement action.

“Consumers who purchase products to treat the novel 2009 H1N1 virus that are not approved, cleared or authorized by the FDA for the treatment or prevention of influenza risk their health and the health of their families,” said Michael Chappell, acting FDA Associate Commissioner for Regulatory Affairs. “In conjunction with the Federal Trade Commission, the FDA has developed an aggressive strategy to identify, investigate, and take regulatory or criminal action against individuals or businesses that wrongfully promote purported 2009 H1N1 influenza products in an attempt to take advantage of the current flu public health emergency.”

Products that are offered for sale to the public with claims to diagnose, prevent, mitigate, treat, or cure infections caused by the H1N1 influenza virus that have not been proven to be safe and effective for these uses must be carefully evaluated. Many of these deceptive products are being sold over the Internet via illegitimate web sites. The operators of these web sites take advantage of the public’s concerns about H1N1 influenza and their desire to protect themselves and their families. These fraudulent products come in all varieties and could include dietary supplements or other food products, or products purporting to be drugs, devices or vaccines. Such fraudulent products will not prevent the transmission of the virus or offer effective treatments against infections caused by the H1N1 influenza virus.

“The last thing any consumer needs right now is to be conned by someone selling fraudulent flu remedies,” said FTC Chairman Jon Leibowitz. “The FTC will act swiftly against companies that resort to deceptive advertising.”

Consumers are urged to contact their health care providers or legitimate medical supply services if they have questions or concerns about medical products or personal protective equipment. Consumers are also urged to visit the FDA and Centers for Disease Control and Prevention web sites for more information about this emergency, and to determine which products the FDA has approved, cleared or authorized for use to diagnose, treat, prevent, mitigate or cure infections caused by H1N1 influenza virus.

Consumers should also visit FDA's web site for tips about how to protect themselves when buying medicines online: http://www.fda.gov/buyonlineguide/

The two antiviral drugs approved by the FDA for treatment and prophylaxis of the 2009 H1N1 influenza virus are Tamiflu (oseltamivir phosphate) and Relenza (zanamivir). Tamiflu and Relenza, in addition to their approved labeling, have Emergency Use Authorizations that describe specific authorized uses during this public health emergency.

For more information about FDA-approved antiviral drugs for influenza, see http://www.fda.gov/cder/drug/antivirals/influenza/default.htm.

For more information on CDC recommendations regarding use of antiviral drugs against the current novel 2009 H1N1 influenza strain, see http://www.cdc.gov/swineflu/?s_cid=swineFlu_outbreak_001.

For more information about personal protective equipment see http://www.fda.gov/cdrh/ppe/.

At present, there are no licensed vaccines approved for this new H1N1 influenza virus.

Consumers are urged to report any suspected fraudulent products or criminal activity relating to FDA regulated products associated with H1N1 Flu Virus (Swine Flu), including the names of web sites that may be offering these products for sale, to the FDA by visiting: http://www.fda.gov/oci/flucontact.html

No rationale for travel restrictions

venerdì 1 maggio 2009 18.48
-WHO is not recommending travel restrictions related to the outbreak of the influenza A(H1N1) virus. Today, international travel moves rapidly, with large numbers of individuals visiting various parts of the world. Limiting travel and imposing travel restrictions would have very little effect on stopping the virus from spreading, but would be highly disruptive to the global community.

Swineflu (H1N1) updates as they appear

WHO ALERT LEVELS
In Phase 1 no viruses among animals reported to have caused infections in humans.

In Phase 2 animal flu virus is known to have caused infection in humans and is potential pandemic threat.

In Phase 3, animal or human-animal flu virus has caused small number of cases in people, but not sufficient for community-level outbreaks.

Phase 4 is characterized by verified human-to-human transmission of animal or human-animal flu virus, indicating greater but not inevitable risk of pandemic.

Phase 5 means human-to-human spread of virus is confirmed in two countries in one WHO region and pandemic is feared imminent.

Phase 6 characterized by community-level outbreaks in at least one other country in second WHO region. This phase indicates global pandemic is under way.

FDA Authorizes Emergency Use of Influenza Medicines, Diagnostic Test in Response to Swine Flu Outbreak in Humans

The U.S. Food and Drug Administration, in response to requests from the U.S. Centers for Disease Control and Prevention, has issued Emergency Use Authorizations (EUAs) to make available to public health and medical personnel important diagnostic and therapeutic tools to identify and respond to the swine flu virus under certain circumstances. The agency issued these EUAs for the use of certain Relenza and Tamiflu antiviral products, and for the rRT-PCR Swine Flu Panel diagnostic test.

The EUA authority allows the FDA, based on the evaluation of available data, to authorize the use of unapproved or uncleared medical products or unapproved or uncleared uses of approved or cleared medical products following a determination and declaration of emergency, provided certain criteria are met. The authorization will end when the declaration of emergency is terminated or the authorization revoked by the agency.

Currently, Relenza is approved to treat acute uncomplicated illnesses due to influenza in adults and children 7 years and older who have been symptomatic for less than two days, and for the prevention of influenza in adults and children 5 years and older. Tamiflu is approved for the treatment and prevention of influenza in patients 1 year and older.

The EUAs allow for Tamiflu also to be used to treat and prevent influenza in children under 1 year, and to provide alternate dosing recommendations for children older than 1 year. In addition, under the EUAs, both medications may be distributed to large segments of the population without complying with the label requirements otherwise applicable to dispensed drugs, and accompanied by written information pertaining to the emergency use. They may also be distributed by a broader range of health care workers, including some public health officials and volunteers, in accordance with applicable state and local laws and/or public health emergency responses.

In authorizing an EUA for the rRT-PCR Swine Flu Panel diagnostic test, the FDA has determined that it may be effective in testing samples from individuals diagnosed with influenza A infections, whose virus subtypes cannot be identified by currently available tests. This EUA allows the CDC to distribute the swine flu test to public health and other qualified laboratories that have the needed equipment and the personnel who are trained to perform and interpret the results.

The test amplifies the viral genetic material from a nasal or nasopharyngeal swab. A positive result indicates that the patient is presumptively infected with swine flu virus but not the stage of infection. However, a negative result does not, by itself, exclude the possibility of swine flu virus infection.

The EUA authority is part of Project BioShield, which became law in July 2004.
